Registration Form Introduction

You have to use PASSWORD for entering the form.

Form needs to be filled with the following information:

FIRST PAGE:

State which you represent
Name and Surname
Gender
Date and place of Birth
Citizenship
Residence (address)
Passport number
E-mail address for contact
Personal Code (Code have to be given to you when your application is accepted - if you don't have a Code, let us know)
Personal photo (upload)
Button "NEXT"

Here you have to choose your ART CATEGORY

Here you have to choose your ARTISTIC DISCIPLINE

On the top of the page is the name of your chosen Category / with Discipline

Below that you have to confirm with one "click" on the "box" in front the sentence "I will participate in this discipline" if this is the correct Discipline.

(If you made a mistake, you can scroll down page, and make a "click" on to go back. Don't use any other "back" buttons, or you will need to start again, from the beginning.)

Then you can add information about your exhibit/s (photo, name, year, dimensions, technique), or/and other requested data.

Then you have to "click" on offered "back" button, below

If you will participate in only one discipline, and your application is complete, "click" offered "back" button until you get to the "SUBMIT" BUTTON

BY CLICKING ON SUBMIT BUTTON - YOU WILL REGISTER YOURSELF FOR THE GAMES-2013.

You will recieve a copy of the information you have sent via the email address you provided on the first form page.
